Testing for COVID-19? Nova Scotia Health reminds you to fill out online screening tool to ensure you are considered for COVID-19 medication
Nova Scotia Health urges Nova Scotians to complete the Report and Support online screening tool as soon as they book a PCR test or have received a positive rapid test result.
This online screening tool assesses a person’s risk for serious illness due to COVID-19, and their eligibility for medications that could reduce the risk of hospitalization. This is particularly important for people in their 60s and older as they are considered to be at a higher risk for serious illness.​
Medications for early and mild COVID are recommended within 5 to 7 days of symptom onset. Providing information in the report and support form in advance will allow timely assessment for medication/treatment for those with a positive PCR result.
Information from the form is also used to determine eligibility for other supports, such as from the COVID Community Virtual Care Team, or Public Health.
If a person tests negative on a PCR test, their information will not be used and will be deleted. If a person has another PCR test in the future, they need to complete the Report and Support screening tool again.

Those who do not have internet access can call 1-833-797-7772 to have the form completed.
